Current methods of treating chronic diarrhea have limited efficacy and have some side effects. Probiotics have the potential to reduce the symptoms of diarrhea.

However, the lack of evaluation of blood parameters and the short duration of the intervention were limitations of the study.

Study Randomized, Double-Blind, Placebo-Controlled Trial Evaluating ProbioticsLactobacillus plantarumTherapeutic effect of P9 strain on young patients with chronic diarrhea (clinical trial registration number: ChiCTR2000038410). Dry expected 28d, 14d after intervention. Participants were randomized to P9 (n = 93) and placebo (n = 96), with 170 completing the double-blind intervention phase (n = 85 per group). The primary endpoint was the diarrhea symptom severity score. Both intention-to-treat (n = 189) and protocol analyses (n = 170) showed a modest but statistically significant reduction in diarrhea severity compared to placebo (20.0%, P = 0.050;21.4%, P = 0.048).

Chronic diarrhea is a common functional gastrointestinal disorder affecting 3-20% of the global population. It is defined as having more than 3 bowel movements per day or loose stools for at least 4 weeks. Common causes of chronic diarrhea includefunctional diseaseThere are many clinical and social problems for patients. At present, the treatment of chronic diarrhea is mainly drug therapy. However, these drugs can cause side effects such as constipation, nausea, vomiting, bloating and further diarrhea. In addition, there are concerns about antibiotic resistance and persistence associated with antibiotic use. Therefore, it is very difficult to treat chronic diarrhea. People with diarrhea tend to have reduced gut microbial diversity, which increases their susceptibility to pathogens. Many microbially-derived metabolites, including bile acids (BAs), short-chain fatty acids (SCFAs), and amino acids, are linked to gastrointestinal inflammation and colonhomeostasisRelated. Recent research has also focused on intestinal bacteriophages, which can alter the gut microbiota and affect gastrointestinal diseases such as inflammatory bowel disease and colorectal cancer. Therefore, the gut microbiota, metabolome, and phage are considered targets for the treatment of diarrhea.

Probiotics are defined as "live microorganisms that, when given in sufficient amounts, confer health benefits to the host," such as reducing disease symptoms and improving gut microbiota balance. They have been used to control diarrhea, relieve symptoms by reducing inflammation, restore the balance of intestinal flora, and enhance intestinal mucosal barrier function. While some trials involving single or multiple strains of probiotics have shown improvements in symptoms and quality of life in patients with chronic diarrhea, particularly in bowel frequency, stool consistency, and health status, studies have reported that in IBS-D patients, probiotic fermented milkThere was no improvement in gastrointestinal and psychological symptoms or quality of life after 8 weeks of intervention. These differences emphasize that the beneficial effects of probiotics on diarrhea are influenced by various factors. Further studies and microbiological analyses are needed to describe their functional role.Probiotics have shown good effects in improving intestinal homeostasis, which can lead to a series of multiple and reversed functional effects.

Lactobacillus plantarum P9 is a potential probiotic isolated from traditional acidic gruel. The P9 strain has been found to possess several desirable properties, including high tolerance to gastrointestinal fluids, potent immunomodulatory, anti-inflammatory, and gut microbial homeostasis-regulating activities. Taking P9 can relieve symptoms of chronic constipation and is accompanied by an increase in functional gut microbiota and microbial metabolites. Unlike traditional drugs, which usually have a one-way effect, the role of probiotics is to restore the balance of the gastrointestinal tract.Given the multifaceted role of probiotics, it is interesting to investigate whether P9 also has anti-diarrhea properties.

The purpose of a large, randomized, double-blind, placebo-controlled clinical study was to evaluate the efficacy and safety of P9 in the treatment of chronic diarrhea over a 4-week period. The primary endpoint was diarrhea symptom severity score, while secondary endpoints included stool consistency, frequency, urgency, gut microbiota, and metabolomic changes. Studies have shown that the intake of P9 has a certain alleviation effect on chronic diarrhea. Importantly, the study also revealed desirable changes in the fecal microbiome and metabolome after P9 treatment. These findings highlight the subtle role of P9 in regulating gut health and support further exploration of probiotics as a viable strategy for the treatment of chronic diarrhea.
