On August 12, 2024, Katherine Kedzierska of Melbourne University, Brendon Y. Chua, Wang Zhongfang of Guangzhou Medical University and Jeremy Chase Crawford of St. Jude Children's Research Hospital jointly communicatedCell Published online entitled"High expression of oleoyl-ACP hydrolase underpins life-threatening respiratory viral diseases"'s research paper,In patients hospitalized with avian influenza A (H7N9), the study investigated early drivers of the fatal disease.Transcriptomics and the enzyme that mediates fatty acid production, oleoyl-carrier proteolytic enzyme (OLAH), is strongly associated with lethal A/H7N9 virus early after admission and continues until death. The expression of OLAH was lower in the rehabilitation patients during hospitalization.

In response to life-threatening seasonal influenza, COVID-19, respiratory syncytial virus (RSV) andmultisystem inflammatory syndrome in children(MIS-C) High levels of OLAH were also detected in hospitalized patients, but not in mild disease. in olah-/-In mice, lethal influenza infection resulted in survival and mild disease, and reduced lung viral load, tissue damage, infection-driven lung cell infiltration, and inflammation. This is induced by different lipid droplet kinetics and reduced viral replication andmacrophage inflammationsupported. Supplementation of oleic acid, a major product of OLAH, increases influenza replication and its inflammatory potential in macrophages.Together, the study's findings determine how the expression of OLAH drives life-threatening viral diseases.

Respiratory infections cause significant morbidity and mortality and remain the deadliest infectious disease. However, it is not clear why some people die from life-threatening illnesses while others develop mild symptoms. Severe influenza disease results from overactivation and/or perturbation of the immune response, lung tissue damage, and hypercellular media (Hypercytokinemia). Young children, the elderly, pregnant women and people with comorbidities are at high risk of serious illness. Admission may also occur in healthy individuals without prior risk factors. Among patients hospitalized for influenza-like illness during 2015-2016,>60% were previously healthy with no underlying disease. If effective vaccines and treatments are to be designed to control respiratory disease, it is essential to tap into the factors that cause severe influenza illness.

Increased susceptibility to severe influenza disease is multifactorial, with host factors contributing to disease outcome in addition to an impaired or hyperactive immune response and/or a low preexisting immunity.Interferon-induced transmembrane protein -3 (IFITM3) can limit viral replication by preventing endocytosis of the virus into the cytoplasm. However, a single IFITM3nucleotide polymorphismIts function can be eliminated, leading to an increase in the severity of the disease. Mutations in genes associated with Toll-like receptor 3 and regulatory elements that mediate the production of type I and type III interferons increase susceptibility to severe influenza. Regulation of host lipid factors and their metabolism also affects influenza virus replication, inflammation, and disease outcome.Viral infection drives lipidomic changes in the lungs, typically characterized by an increase in fatty acids and lipid mediators, providing energy and resources for virion assembly and contributing to the immune response.

Following the 2013 outbreak of avian influenza A (H7N9) virus in China (with a mortality rate of about 35%), researchers defined immunity to the deadly A (H7N9) virus. Immunoreactive dysregulation in hospitalized patients with fatal A H7N9 compared with recovered patients andhypercellular media. Here, researchers explored the relationshiphypercellular mediamechanisms associated with fatal diseases. The researchers identified a gene that encodes an enzyme involved in endogenous fatty acid production, oleoyl-carrier proteolytic enzyme (OLAH), an early driver of fatal outcomes during H7N9. OLAH levels and life-threateningseasonal influenza virus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2),respiratory syncytial virus(RSV) infection is associated with multisystem inflammatory syndrome (MIS-C) in children. OLAH regulates lipogenesis by catalyzing the release of fatty acids from acyl carrier proteins following carbon chain elongation within the fatty acid synthase complex.OLAH released different chain length fatty acids at the same time, oil-ACP23 to produce oleic acid with high catalytic efficiency.
To date, the role of OLAH in regulating immunity and driving disease outcome in any disease has not been studied. The study found that in patients with A(H7N9) fatal disease, OLAH gene expression was significantly elevated early in the disease and remained high until death. High OLAH levels were found in four additional groups of patients hospitalized for life-threatening seasonal influenza, COVID-19, RSV infection and pediatric patients hospitalized for MIS-C.CRISPR-Cas9-mediated olah knockout in mice reveals the mechanism of OLAH-mediated disease severity.
infected olah with a lethal dose of influenza-/-mice, resulting in survival, mild disease, reduced viral load in the lungs, tissue damage, infection-driven pulmonary innate cell infiltration, and inflammation. This is associated with differences in lipid droplet dynamics, reduced macrophage viral infection, and an inflammatory environment. Inhibition of lipid droplet formation reduces viral infection in macrophages, while supplementation of oleic acid, the major product of OLAH, increases influenza viral infection and inflammation in macrophages.Together, the study's findings provide mechanistic insights into how expression of OLAH drives life-threatening respiratory diseases.
